Treatment of diseases of the endocrine system

Treatment of diseases of the endocrine system

Regulation of the endocrine function of the reproductive system begins with the cerebral cortex and through the thalamus-hypothalamus-pituitary-adrenal chain reaches the actual reproductive organs (uterus, mammary glands).

The service is available in branches:
  • Odessa, Sudnobudivna, 1B
  • Kyiv, Kostiantynivska, 57
Treatment of diseases of the endocrine system

Everything that happens around us primarily affects the cerebral cortex and is transmitted further down the chain, so over the past 9 years, in the clinical practice of our doctors, cases of stress amenorrhea (absence of menstruation) have significantly increased. But stress for the body is not always shelling, being in a bomb shelter or losing loved ones, the state of the endocrine system is also affected by, for example, lack of sleep.

Our capabilities in the treatment of diseases of the endocrine system in gynecology

To find out the cause of a reproductive disease, the gynecologist-endocrinologist must first talk with the patient about the peculiarities of her life, previous diseases, family history, etc. You should spend at least half an hour on the first meeting, and preferably an hour. And after that, you can order an examination: first of all, an ultrasound of the pelvis, mammary glands, and thyroid gland. All this can be done on site at the VIRTUS Institute, because we have specialist doctors and the necessary equipment.
It is very important to take into account the day of the cycle: usually the examination is scheduled for the 5-9th day from the start of menstruation. Thus, a woman on the first day of menstruation can call and make an appointment for a week, come to the doctor and get a full examination in one day.
The next step is to examine the level of hormones in the first (day 3-5) and second (day 21-23) phases of the menstrual cycle. If the cycle is longer or shorter than 28-30 days, the examination dates will vary. With the results of hormone level analysis and ultrasound, the patient comes for a second consultation, during which the doctor offers a treatment plan. Sometimes additional research is needed: for example, when a woman has a high level of prolactin. In 20-30% of cases, it may be associated with changes in the pituitary gland, in particular the formation of a micro- or macroadenoma, and then magnetic resonance imaging of the head, preferably with contrast, is necessary. But you should not do any examinations before consulting a gynecologist: this is an unnecessary waste of time and money.
Usually, 1-2 months after the start of treatment, depending on the diagnosis, a control ultrasound and an analysis of the level of hormones (individual types or a complete package) are prescribed.

Treatment of diseases of the endocrine system in gynecology

Irregular menstrual cycles, whether they are too long, too short, or delayed, are often treated with the standard regimen of birth control pills. But there are many causes of cycle disorders, so such therapy is appropriate in about 30% of cases. Before prescribing treatment, it is not enough just to pass a general blood test, liver tests and do an ultrasound. A gynecologist must know diseases of the endocrine system, understand the chain of endocrine function of the reproductive system. The cause of problems with menstruation can be hypothyroidism or hyperthyroidism (insufficient or excessive function of the thyroid gland), and therefore the patient should be referred for an analysis of thyroid hormones and prolactin. Prolactin is one of the stress hormones, and at high levels it can trigger amenorrhea. Increased production of prolactin causes not only stress – it is enough to work a night shift or go to bed late, because the release of hormones in people is subject to daylight. It is also important that the chain of functions of a woman’s reproductive system is not designed for the number of periods she has. After all, menstruation is a pregnancy that did not occur, and during the life of our ancestors, a woman had 50-60 menstruations, not 500-600, as today.
Thus, the first task of a gynecologist-endocrinologist is to understand where the deviation from the norm occurs and what provokes it.

Popular questions

How to restore the hormonal background?

It is necessary to restore the hormonal background when there is a violation, but first the doctor must find out what exactly is damaged. If the thyroid gland is not working well, it may be enough to take drugs with iodine or selenium, sometimes hormonal therapy is necessary in the treatment of the endocrine system. First of all, you should go to bed on time, have a normal body mass index, moderately engage in physical activity, sometimes watch the news a little less to limit stress

What hormones should be taken before a visit to a gynecologist-endocrinologist?

Before the initial consultation, you should not take any tests, they are prescribed by the gynecologist based on the results of the conversation. If there are examination results after the previous visit, you should take them with you.

What is a gynecologist-endocrinologist responsible for?

A gynecologist-endocrinologist treats disorders of the menstrual cycle, skin problems and the inability to get pregnant, caused by disorders of the endocrine system. He must understand how the thalamus-hypothalamus-pituitary-adrenal gland-reproductive system chain works, and identify exactly where the pathology occurs and prescribe therapy.

When should you consult a gynecologist-endocrinologist?

You should consult a gynecologist-endocrinologist when you notice changes in your condition that are alarming:

  • weight loss without making any effort may indicate thyrotoxicosis or cancer;
  • changes in the regularity and duration of the cycle;
  • changes in the amount of bleeding;
  • amenorrhea;
  • absence of pregnancy for more than a year;
  • the appearance of secretions from the mammary glands without pregnancy and lactation.

Services and procedures in the treatment of diseases of the endocrine system in gynecology

Problem skin, infertility, menstrual cycle disorders are an incomplete list of the main complaints with which women turn to a gynecologist-endocrinologist.

Rashes on the face are one of the symptoms of endocrine dysfunction, and 90% of such patients manage to help improve their appearance. — Says gynecologist-endocrinologist Serhiy Ovcharenko. — First of all, we are talking about hyperandrogenic conditions of ovarian origin, which are successfully corrected by taking birth control pills. But in 30-40% of cases, the problem lies in the adrenal glands, and then the pills do not cure, but only mask the problem, so you have to choose other treatment options. In rare cases, about once a year, I diagnose Itsenko-Cushing syndrome, which is a reason to immediately contact the Kyiv Institute of Endocrinology – this is a serious condition that, without correction, threatens to be fatal. Many patients come because of long-term absence of pregnancy. I am very happy when I manage to identify a problem and find a solution, I have my own portfolio of children whom I helped to come into the world. But sometimes I have to explain to a woman that she should turn to reproductive specialists, because this is a completely different specialty.

The advantage of the VIRTUS Institute is that we can perform a complete examination necessary to determine the cause of endocrine disorders in gynecology. In particular, we offer the following services and procedures for the diagnosis and treatment of diseases of the reproductive and endocrine system:

  • determining the level of hormones in the blood in the first and second phases of the menstrual cycle;
  • study of the function of the thyroid gland, adrenal glands, liver;
  • endocrinological studies: determination of blood glucose and insulin levels
  • examination on a gynecological chair and extended colposcopy;
  • Ultrasound of pelvic organs, mammary glands, thyroid gland;
  • hysteroscopic examination of the uterine cavity and biopsy of the endometrium followed by histological examination.

In fact, menstrual irregularities, infertility, and skin rashes are related, and we understand the causes and help women solve their health problems. As a rule, we succeed because we have a lot of experience and we continue to study and follow modern treatment trends.

Gynecologist-endocrinologist consultation

The initial consultation of an endocrinologist-gynecologist at our Institute lasts about an hour. Sometimes all this time is needed to collect anamnesis and identify the source of the problem. A more optimal distribution of time is half an hour for a conversation and half an hour for a planned examination in the gynecological chair and colposcopy (if necessary). To make the visit to the gynecologist more fruitful, the patient should prepare answers to the following questions in advance:

  • date of birth;
  • the date of the start of the first menstruation: for Ukrainian women, this is mostly the age of 12-13 years;
  • the interval from the beginning of one to the beginning of the next menstruation: the norm varies from 21 to 39 days, but if the intervals are different each time, this already indicates a problem, even when each individual value fits within the normal range;
  • how long the menstrual bleeding lasts – again, not only the number of days is important, but also the regularity;
  • the day of the start of the last menstruation – currently there are many mobile applications that allow you to track the cycle;
  • the date of the last visit to the gynecologist (it is advisable to bring all the results of previous examinations and discharge with you);
  • the number of pregnancies since the beginning of the cycle, including abortions;
  • what diseases did blood relatives have (somatic, hypertension, diabetes, cancer, etc.);
  • what were you sick with;
  • who had operations during their life (septoplasty, implants, hysteroscopy are also taken into account).

Consultation of a gynecologist-endocrinologist is not necessarily carried out every year, if the woman is generally healthy.
